Statement of Purpose: This position is responsible for providing
psychosocial evaluations, individual, couples, family and/or group
therapy to all CAN Community Health mission patients and our
community partners.
Essential Functions: Mental Health Counselor
- Diagnose and treat mental health disorders using evidence-based
treatment modalities.
- Collects data about patients through interview, case history,
psychological tests, and/or observational techniques; evaluates
data to identify cause of problems and to determine proper
therapeutic approach or referral to other resources.
- Creates individualized treatment plans according to patient
needs and circumstances.
- Meets with patients regularly to provide counseling and adjust
treatment plans as necessary.
- Provides therapeutic crisis intervention as required.
- Collaborate with other staff and multidisciplinary team members
to perform clinical assessments and develop treatment plans.
- Documents therapy visits and all encounters in a timely manner
in the electronic medical records system. Maintain up-to-date
client and program records as required by funders and agency.
- Evaluates patient progress and makes individual
modifications.
- Act as a client advocate to coordinate required services.
- Guide clients in the development of skills and strategies for
dealing with their problems.
- Counsels' patients, individually or in group sessions to assist
them in overcoming dependencies, adjusting to life, and making
changes.
- Modify treatment activities and approaches as needed to comply
with changes in patient's status.
- Follows all safety protocols and maintains client
confidentiality.
- Accurately completes and submits billing documentation for each
patient encounter.
- Adheres to ethical and legal standards of professional
practice.
- Refers patients to outside specialists or agencies when
necessary.
- Contributes to CAN Community Health clinics by accomplishing
related tasks as needed.
- Promotes and practices CAN Community Health Inc's mission and
values and follows its policies and procedures.
- Ensures confidentiality is maintained by entire team regarding
patient/client information in accordance with HIPAA, professional
and departmental standards.Secondary Tasks: Mental Health
Counselor
- Attends and participates in Behavioral Health team meetings as
appropriate.
- Attends and participates in Interdisciplinary team meetings as
appropriate.
- Assists in composing, evaluating, revising, and implementing
Behavioral Health policies.
- Pursues professional development training.
- Performs all other job-related duties as assigned.
Requirements:
Education/Professional: Mental Health Counselor
- Master's degree in psychology, social work, counseling, or
related field.
- Current with all state licensing requirements, including LCSW,
LMHC and LMFT.
- Three years of direct clinical practice providing therapy in a
medical clinic, hospital, or private practice.
- Experience working with co-occurring populations (HIV, chronic
health conditions, mental health, and substance use.)

Work Environment: Mental Health Counselor
- This job operates in a professional office environment. This
role routinely uses standard office equipment such as computers,
audio visual, telephones, photocopiers, filing cabinets and fax
machines.
Physical Requirements: Mental Health Counselor
- Neat, professional appearance.
- Sedentary- Exerts up to 10 lbs. of force occasionally and/or a
negligible amount of force frequently or constantly in lift, carry,
push, pull, or otherwise move objects. Involves sitting most of the
