It is our expectation that each employee will offer his/her full capabilities wherever and whenever necessary to ensure the success of our endeavors.*JOB DESCRIPTION*Summary:The Registered Nurse is a member of a clinical health care team that providing routine patient care under the supervision of a licensed clinician. Provides nursing services to patients in a community health setting. Assists in providing education to the patients/family as needed. Responsible for the flow of patients through the medical service areas and exercises frequent independent judgment. Performs with minimal supervision._Launch your career with less debt. You could be eligible to receive up to 60% of your total outstanding qualifying education loan balance through HRSAs Nurse Corps loan repayment program._*Essential Functions:*Case Management* Provides one-to-one chronic disease care management to health center patients/families.* Assesses the patients needs and develops a comprehensive care management plan collaboratively with the patient/family that will determine specific objectives, goals and actions designed to meet the needs identified in the assessment process.* Implements the comprehensive care management plan including interventions to include patient/family counseling and education on disease self-management that will lead to accomplishing the goals of the plan.* Coordinates resources necessary to accomplish the goals in the patients case management plan including referrals to specialists if identified.* Acts as patient advocate by helping others to understand the patients health care situation and health care needs.* Provides referrals to appropriate community resources, facilitates access and communication when multiple services are involved, monitors activities to ensure that services are being delivered and meet the needs of the client, and coordinates services for the purpose of integration and avoiding duplication.Patient Education* Educates patient and/or family on prescribed treatment regimens.* Educates patient and/or family on disease process when newly diagnosed.* Educates the patient on navigating the primary health care system, the importance of compliance with appointments and medications, and self-management of his/her healthcare and/or disease process.* Assists with development and implementation of patient educational materials.* Improves healthcare delivery.Electronic Health Record (EHR)* Accurately and efficiently enters documentation into the EHR. Organize and/or facilitate support groups.* Ensures necessary patient medical information is available in the EHR prior to visit.* Demonstrates basic understanding of the EHR.* Make appointments.* Patient insurances.* Order entry and workflow.* Workflow dashboard.* Document and task management.* Problem flowsheets.* Troubleshooting documents in the clinical inbox.Provider Support* Follows nursing policies and procedures.* Assists with procedures.* Assists with use and management of EHR.* Supports provider with completion of electronic order entry.* Assists providers with patient flow and management.QI/QA* Actively participates in quality improvement projects.* Complies with center policies and procedures; promotes and complies with NCQA PCMH standards of care.* Assists in identification, evaluation, and intervention of quality issues in the clinical setting.Clerical Duties (assist as needed)* Performs clerical duties as directed by Site Supervisor:* Schedule appointments.* Take phone messages.* Scans information into the electronic health record or EHR.* Performs patient registration functions.* Accurately pre-screens medical/dental records.* Assists with document management.* Supports patients with referral process.Leadership* Assesses patients for appropriate department or referral to the ED.* Assists with management of patient flow to assure effective and efficient care is provided to all patients.* Orders and maintains inventory of medications.* Maintains the sample and pharmacy log.* Checks for expiration dates and disposes of expired medications following center policy.* Assures proper ordering and maintenance of medical supplies* Assures medical equipment is reprocessed and sterilized per established policies and procedures.* Assists for completion of monthly reports* Performs other related duties as indicated or when requested by supervisor.* Covers for direct supervisor as needed.Assist MA/LPN as needed:* Assures patients are worked up for provider visit timely and efficiently to include:* Obtaining accurate vital signs from the patient and reports abnormal findings to provider.* Accurately documenting patients health history and reconciles medications.* Accurately completes patient information on laboratory orders and referrals.* Keeps exams rooms and treatment areas clean per infection control policy and procedures and well stocked.* Fills in logbooks and special forms.* Performs CLIA waived testing as ordered by the provider.* Follows policies and procedures.* Enters results per policies and procedures.* Reviews logbooks and reports monthly for accuracy and completion* Performs phlebotomy and lab collection as needed.* Prepares specimens for transport to performing labs appropriately.* Accurately utilizes medical equipment as trained.* Administers medications by oral, intramuscular, subcutaneous, intradermal, inhaled routes as directed by provider.* Assists in referral process.* Enrolls patients in pharmaceutical assistance programsOther* Appropriately applies nursing process to clinical situations.* Always maintains use of universal precautions.* Completes daily/day end activities as assigned.* Completes all required trainings and attends meetings as assigned.* Protects/observes patient confidentiality per policies and procedures.* Assures translation, as needed, for non-English speaking patients.* Recognizes and responds appropriately to violent/abusive situations, bomb threats, fire and emergency situations.*Minimum Qualifications: (Experience/Skills/Abilities/Education/Certifications)** Graduate of an accredited school or college of nursing.* Current State of Florida Licensed Registered Nurse.* Demonstrates good oral and written communication skills.* Ability to effectively interact with physicians, patients, and other staff members.* Possesses intermediate typing skills.* Demonstrates knowledge and ability to use Office 365 programs including Outlook, Word, and Excel.* Demonstrated knowledge of proper, safe, and efficient use of current office equipment/software.* Effectively uses the electronic health record.*Preferred Qualifications: (Experience/Skills/Abilities/Education/Certifications)** Two years of Nursing experience preferred.* Bilingual communication preferred.*Physical Demands: *While performing the duties of this job, the employee is regularly required to stand; walk, often for prolonged periods; use hands to finger, handle, or feel; reach with hands and arms; stoop, kneel, crouch, or crawl; and see, speak, and hear.
