Job Description:

Aveanna Healthcare seeks a Physical Therapist to provide home health care services to patients in need of recuperation from illness or injury.
Key responsibilities include:

• 
  
• Conducting initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
  
• Administering, evaluating, and interpreting tests and measurements to determine functional goals and outcomes.
  
• Developing and implementing plans of care to meet physical therapy needs, goals, and progress.
  
• Delivering physical therapy services in accordance with prescribed physician orders.
  
• Maintaining accurate documentation of client performance, modifying goals and treatment as needed, and determining discharge care plans.
  
• Collaborating with healthcare teams, physicians, nurses, and family members to ensure effective care.
  
• Supervising other personnel as applicable.
  
• Completing OASIS assessments where allowed by state professional practice and regulation.

    Requirements:
    
    • 
      
    • Degree in Physical Therapy from an accredited university/program.
      
    • Current, unrestricted state license as a Physical Therapist in the state of practice.
      
    • Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
      
      Preferred qualifications:

      • Strong organization and communication skills.

      • Proven relationship-builder.
        
      • Experience in Home Health Therapy and completion of OASIS documentation.
