About the Role:

Tidewell Hospice is seeking a Social Worker to support patients and families during their end-of-life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve.

Key Responsibilities:

* Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
* Provide individual and family counseling, crisis intervention, and bereavement support.
* Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
* Assist with financial resources, Medicare/Medicaid, and long-term care planning.
* Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
* Support patients and families with advance directives and memorial planning.
* Educate the community on hospice care.
* Participate in an on-call rotation.

Qualifications:

* Education & Experience: Minimum one year of social work experience in a healthcare setting (internships may qualify).
* Licensure: Must hold one of the following Florida licenses within 90 days of hire:
* Licensed Clinical Social Worker (LCSW)
* Licensed Mental Health Counselor (LMHC)
* Licensed Marriage and Family Therapist (LMFT)
* Other Requirements:
* Valid Florida driver's license and reliable transportation for field-based roles.
* Proof of valid auto insurance.
