Job Description:
Encompass Health Corporation is seeking a dedicated and
patient-focused Licensed Practical Nurse (LPN) or Licensed
Vocational Nurse (LVN) to join our integrated network of inpatient
rehabilitation hospitals and home health agencies. In this role,
the LPN-LVN will provide compassionate nursing care to a diverse
population of patients, supporting our mission of setting the
standard in quality rehabilitation and home-based care. This
position is crucial in facilitating the well-being of patients and
helping them achieve their highest level of independence.
Administer nursing care to ill, injured, or disabled patients
within a rehabilitation environment or home setting. Monitor
patient health status and response to treatment, and modify care
plans as needed. Ensure the accurate implementation of
physician-directed care plans including medication administration
and wound care. Collaborate with a multidisciplinary team to
facilitate coordinated and comprehensive care. Educate patients and
their families on illness management, prevention, and post-hospital
treatment. Maintain accurate patient records and report any changes
to the supervisory medical professional. Proficient in providing
patient-focused care Able to administer medications and perform
treatment as prescribed Excellent interpersonal and communication
skills Adept at maintaining comprehensive and accurate patient
records Capable of working both independently and collaboratively
in a team environment Medical Insurance Dental Insurance Vision
Insurance 401k Current and unrestricted LPN or LVN license in the
state of practice. CPR certification required.
