Registered Nurse (RN) | Case Manager Location: Cape Coral, FL Agency: Lancesoft Inc Pay: $2,300 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/23/2026 About the Position TravelNurseSource is working with Lancesoft Inc to find a qualified Case Manager RN in Cape Coral, Florida, 33904! Shift 8-Hour Days – 8:00 AM to 4:30 PM 40 guaranteed hours per week Weekend rotation: 1 weekend per month No on-call Floating required (travelers float first; may float to sister campuses) Facility: Cape Coral Hospital Key Responsibilities – Case Manager RN (Care Management) Perform acute care case management for hospitalized patients Complete admission and continued stay reviews Coordinate discharge planning (home health, SNF, rehab, DME, etc.) Collaborate with physicians, nursing, social work, and interdisciplinary teams Ensure compliance with medical necessity and payer guidelines Facilitate smooth transitions of care to reduce readmissions Document accurately and timely in the EMR Function as core staff and integrate into unit culture 5 Benefits of Travel Nursing As a travel nurse, you'll work in various healthcare settings, gaining exposure to diverse patient populations, medical procedures, and healthcare systems.
