Description:Position Summary:Ensures continuity of care to clients
through the transition from homelessness into housing. Encourages
sustained recovery by providing practical and emotional support and
empowerment to clients. Responsible for delivering homeless
outreach services (e.g. providing supplies, transportation).Minimum
Qualifications:High school diploma or GED required.Lived experience
with homelessness, Substance Use Disorder (SUD) and/or Co-occurring
disorder (COD), or experience gained by personal/family member
experience/diagnosis. Experience successfully navigating the
mental/physical healthcare systems and other community support
systems/resources. Also requires knowledge of community services.
Strong communication, ability to work on a team and time management
skills necessary. Valid Florida driver's license with reliable
transportation, a good driving record, and valid auto
insurance.Preferred Qualification: Bi-lingual skills preferred.
Bachelor's Degree in Social Work, Psychology, or related field with
minimum 2 years' experience preferred. Value cultural differences
and be genuinely interested in working with people experiencing
homelessness, SUD, and/or COD.Special Skills, Knowledge and
Abilities: Must possess excellent communication and listening
skills, creative problem solving skills, and ability to coordinate
multiple tasks. Must be able to create a culture of respect and
have a natural ability to establish/build rapport with clients and
the ability to demonstrate wisdom and skills learned from personal
coping/recovery. Knowledge of community services and resources.
Highly motivated, self-directed. Willingness to work on a treatment
team. Ability to build professional relationships in the community
and networking.Essential Functions/Job Duties: Conduct intensive
and assertive outreach in shelters/encampments/soup
kitchens/resource centers/other areas frequented by people
experiencing homelessness.Interface with homeless-serving
organizations, state/local agencies, healthcare providers, law
enforcement, etc. to identify/refer/engage individuals in need of
services.Expand referral network and build community resource
inventory Assist with developing/delivering training materials and
with stakeholder training/education.Coordinate with project care
team to enroll clients and establish Individual Care Plans
(ICPs).Build rapport and establish trusting relationships with
clients.Provide practical/emotional/informational support to assist
participants in accessing recovery support services, developing
positive social connections, and learning to manage their
recovery.Model effective coping techniques and self-help
strategies.Assist clients in identifying personal recovery goals,
setting objectives, and maintaining self-care.Inputs clinical
documentation into Avatar on a daily basis and in a timely manner
for all services rendered directly with or on behalf of clients.
(must be documented by the next business day).Participate in all
required agency, SOAR, and department trainings and related duties
as assigned by supervisor.Work Environment/Physical Demands: This
position operates in both an office environment and community based
settings. Duties of this job are often provided in the place where
the client resides or needs the service. This position may be
subject to exposure to individuals who engage in maladaptive
behavior.Position Type, Expected Hours of Work and Travel: This is
a full time, 40 hour position. Typical hours of work are Monday
through Friday. This position requires flexibility in work hours to
include some evening and weekend hours as needed to meet the needs
of the clients. Frequent local travel is required.Please note this
