They have a current opening for a Hospice Social Worker for their Port Charlotte, FL and surrounding areas . Hospice or Home Health experience is ideal but Social Workers with 1 year of experience in another setting and interest in Hospice care will be considered. These are full-time positions with benefits. Hourly rate depends on experience. Hospice Social Worker Job Requirements 1 years of Social Worker experience in any setting. Hospice or Home Health experience strongly preferred but not required. Florida Licensed Clinical Social Workers, Licensed Mental Health Counselors or Licensed Clinical Marriage and Family Therapist; Florida Registered Interns welcome to apply. Reliable transportation – there is mileage reimbursement for home visits. Hospice Social Worker Job Responsibilities Ensure social service needs of the patient and their families is met. Ensure compliance with all state and federal regulations. Must attend in-office staff meetings when required.
